Access Sydney Community Transport is available to those who meet the eligibility criteria.

Our target groups include people who:

  • Have mobility restrictions due to age;
  • People with a disability and their carer;
  • People who are transport disadvantaged.

Special needs groups within this criteria include people:

  • From culturally and linguistically diverse background;
  • Aboriginal and Torres Strait Islander;
  • Financially disadvantaged;
  • Living in remote or isolated areas.

My Aged Care 

My Aged Care has been established by the Australian Government to help you navigate the aged care system which is designed to give people more choice, more control and easier access to a full range of aged care services.

My Aged Care is made up of a website and a national contact centre phone number 1800 200 422. Together they can provide you with information on aged care, whether for yourself, a family member, friend or someone you’re caring for.  You can request Access Sydney Community Transport as your preferred transport provider.

  • If you are 65 years or older, you will need to complete a referral form by visiting www.myagedcare.gov.au or by calling the My Aged Care number on 1800 200 422.
  • If you are under 65 years, please call our office to discuss your needs with one of our Client Liaison Officers.
  • If you are Aboriginal or Torres Strait Islander aged 50 years or older, you will also need to complete a referral form from My Aged Care.
  • If you have already registered with My Aged Care and have nominated ASCT as your preferred provider,  please download the form below.
